Welcome to the Organizational Membership application! If your goal is to provide the best and most affordable benefits program for your employees then you have come to the right place. This page has all the information you need to become a member.

When to Apply:

You may apply at any time to ESM, but be aware that all memberships are January through December regardless of when you join.

Dues:

Dues must be paid at the time of membership registration.  Dues do not change even if you are applying in the middle of the year. For members who wish to join between July and December, you can pay half of the membership fee if you pre-pay for the entire upcoming year.  Members who wish to join between November and December can pay for the upcoming year and get those two months for free.

The chapter accepts all credit card payments.

Application Instructions:

Please download the application from this site and follow all of the instructions. Remember to fill out the form completely. An incomplete application may be returned for additional information and delay the activation of your membership for as much as one month. It is recommended that applicants mail their completed application along with their Primary and Alternate Representative's business cards and payment to the local Chapter at:

When paying by credit card, the application may be faxed to 602-467-3004. The information provided on the application will be published in the member directory and question responses will be used as a marketing tool for the Associate Representatives. At the very least please notify us at admin@esmphx.org of your future membership.  New member packets will be sent to you within the first month of your membership. Renewal notices are sent the first part of November.

Please Note: The primary Organization Representative cannot be the primary representative of any other membership.
