Associate Membership
Welcome:
Welcome to the Associate Membership application page! If you are a company looking to become a supplier of discounted products or services to ESM members then you have come to the right place. Please feel free to contact us with any questions you may have concerning the application process.
When to Apply:
You may apply at any time to ESM, but be aware that all memberships are January through December regardless of when you join.
Dues:
Dues must be paid at the time of membership registration. Dues do not change even if you are applying in the middle of the year. There is a $50 application fee for new Associate Members. For members who wish to join between July and December, you can pay half of the membership fee if you pre-pay for the entire next year. Members who wish to join between November and December can pay for the upcoming year and get those two months for free.

The local chapter accepts all credit cards except American Express.

Application Instructions:
Please download the application from this site and follow all of the instructions. Remember to fill out the form completely. An incomplete application may be returned for additional information and delay the activation of your membership for as much as one month. Send your application, dues, $50 dollar application fee, certificate of business liability insurance and a Primary and Alternate Representative's business cards to the local Chapter at:

New member packets will be sent to you within the first month of your membership. Renewal notices are sent out the latter part of October.

All Associate memberships are reviewed by the Board, which typically meets on the first Tuesday of every month. All required items must be received before the Board will accept an applicant's membership. Associate members may have to be placed on a waiting list if the ratio of Associates to Organization Members exceeds a 1.25:1 ratio. National Associate members are exempt from the ratio rule.

Liability Insurance Requirements
Associates are required to have a minimum of $500,000 in business liability coverage or E&O insurance. Associate Members are required to have a valid certificate of liability insurance on file with the local chapter at all times. A valid certificate of insurance will show the limits of your coverage and the expiration date of the policy. It must specifically show ESM of Greater Phoenix as a certificate holder and must name ESM as an "additional insured." Failure to adhere to this requirement will result in the suspension of your membership.
Discounted Prices
The discounts shown on the application must be specific. Statements like "reduced pricing" or "substantial discounts" are not acceptable. Avoid the use of vague terms like "no hassle pricing". The discount offered should be comparable to other discount/incentive plans that you offer. General public pricing is not acceptable.
